The World Health Organization (WHO) has long been concerned with the spread of diseases among healthcare workers, particularly those working in high-risk areas such as hospitals and clinics. One disease that poses a significant threat to these individuals is the plague, a bacterial infection caused by Yersinia pestis. Despite being an ancient disease, the plague remains a major public health concern, with outbreaks occurring in various parts of the world.

The plague has three main clinical syndromes: bubonic plague, pneumonic plague, and septicemic plague. Bubonic plague is characterized by inflammation of the lymph nodes after a flea bite or scratch from an infected animal, while pneumonic plague is spread through inhalation of droplets from infected humans or animals. Septicemic plague occurs when the infection spreads through the bloodstream.

The transmission routes for the plague are varied and can include bites from infected fleas or animals, handling or ingesting infected animals or humans, and inhaling aerosolized droplets from infected tissues. The disease has a zoonotic origin, meaning it is primarily spread between animals and humans.

According to the WHO, the main reservoir of Yersinia pestis is rodents, which are responsible for spreading the disease through their fleas. The plague remains a significant threat in many parts of the world, with outbreaks occurring regularly. In fact, it has been categorized as a reemerging disease by the WHO due to its resurgence over the past few decades.

The history of the plague dates back thousands of years and has had a profound impact on human history. One of the most devastating pandemics in recorded history was the Black Death, which occurred in the 14th century and is estimated to have killed between 75 million and 200 million people, roughly one-third of Europe's population.

To inform WHO recommendations on personal protective equipment (PPE) for healthcare workers, it is essential to evaluate whether corpses of plague patients may be infectious. The transmission routes from human corpses are highlighted in Figure 1, which shows the various ways in which the disease can spread.

In conclusion, the plague is a highly infectious and deadly disease that poses a significant threat to healthcare workers and the general population. It is crucial that we continue to monitor outbreaks and develop effective strategies for preventing the spread of this ancient disease.
